N. Sanity Island is the 14th level in Alternate Earth and is based around the first three Crash Bandicoot games on the PlayStation 1.

N.Sanity Island takes place on a jungle-infested, native island around Australia, where the experimental subject and main protagonist Crash Bandicoot lands after his escape from the evil scientist and his creator, Dr. Neo Cortex.
The main gimmick of this level is the different variation of Crash Crates. Normal crates give one Wumpa fruit (reskinned from coins), crates with an "?" icon give 5 Wumpa, the Aku Aku creates give the player a powerup or sub-weapons, crates with the face of Crash give a 1-up, and crates with an up arrow can be used like a spring.
There are also two very dangerous crates in this level: the TNT Crates and the Nitro Crates. The TNT crates will activate a 3-second countdown before exploding after being jumped on, and the Nitro crates will explode immediately upon contact. Both of these crates can cause a chain reaction of explosions around each other as well as defeat any enemy nearby, but they can also hurt the player if they are too close to the radius.
The first area takes place in Turtles Forest, where there are voodoo Tikis of statues, giant mushrooms, a wooden bridge, fire lamps, and a waterfall near. The player will do some platforming and battle some animal enemies, such as big purple lizards and tall green lizards that cannot be stomped on.
The second area is inspired by the level of the Great Gate in Crash 1, as it takes place in a giant, wooden-made dam behind a waterfall as the player finds their way to the top using the arrow springs and the flame lamps. At the end, the player uses a rocket to reach the top of the dam.
The checkpoint area takes place on top of the dam with the entrance to the Temple Ruin. The player can also see the other two islands from afar, the Wumpa Island and the Cortex Island, the home of the evil scientist's castle.
The next two areas take place in the Temple Ruin, an ancient temple that has some traps still active, such as spears that pop up from the floor and falling platforms. The player also has to continually deal with the animal enemies as well as various Mario enemies, such as the Chain Chomps.
After exiting the temple, the player is atop the island in the checkpoint room as the boss fight with Dr. Cortex awaits.
Dr. Cortex attacks with exploding crates and ray gun projectiles from his hoverboard. His force field also protects him from the player's attacks, but he can be harmed by the explosions caused by the crates. The player must carefully dodge Cortex's ray gun attacks and trigger the TNT crates at the right time to damage him whenever he moves. After taking enough damage, Dr. Cortex's hoverboard starts to explode, sending him flying back to his castle. Thus, the level has been cleared for the player.

- The coins in this level are reskinned as Wumpa Fruit, the main currency of the Crash Bandicoot series.
- The background in this level comes from Donkey Kong Country 2.
- The tileset for this level comes from The Lost Vikings 2 with log columns from Claw.
- Koopa Troopas and Chain Chomps were originally absent in this level.
- The Koopa Troopa's appearance in this level is a reference to the turtles enemies in Crash 1 and Crash 2.
- The Chain Chomp's appearance in the temple area is a reference to the snakes enemies that appear in the temple theme levels from Crash 1.
- Prior to the v0.8 update, the first area of this level had a "blue" background issue but has since been fixed.
